A Release Form should be used to request the release of pathology reports (results), recut histology slides, or the release of partial information on a patient or case.

Release form

An Acknowledgement of Custody Form should be used when original slides or paraffin blocks are removed from the BJH archives and may not or will not be returned in their original form.
